Emergency Info & Contact Cards
Immunizations
All new entrants to Kindergarten-Fifth Grades for 2019-2020, must have proof of the following immunizations stamped from a doctor/clinic, NYSIIS, or CIR:
- 3-5 DTPs
- 3-4 Polios
- 3 Hepatitis Bs
- 2 MMRs
- 2 Varicellas (chicken pox).
Requirements for Pre-K students are:
- 4 DTPs,
- 3 Polios,
- 1 MMR,
- 3 Hepatitis Bs,
- 1 Varicella,
- 1-4 HIBs (1 if first is after 15 months),
- 1-4 PCVs along with Lead results.
Requirements for Grades 6-12 are according to NYS PHL 2164.
Medical Modifications
Any child with modifications such as a cast, wheel chair, walker, stitches/staples, a fracture or broken bone, or on crutches or seen in a hospital Emergency Department needs a note to return to school from the health care provider or clinic upon or before return to school. Medical transportation requires a doctor’s note. An updated note from the doctor is required for return to regular physical education and recess.
Medications
MEDICATIONS: Prescriptions, Over the counter medications:
- For safety reasons, Medications that your child needs to take during school MUST be brought to school by an adult
- Medications must be in original packaging and must not be expired
- A doctor’s order is needed
- Parents written consent is needed
- All medications are kept in the Health Office.
If your child has a documented life-threating allergy, I must have an Allergy Action Plan signed by Doctor and written permission from parent along with medication needed. Your child will be excluded from any activity that may put him/her at risk for exposure to allergen including field trips and outdoor recess. If your child no longer is considered at risk, I must have written confirmation signed by your child’s Doctor.
END OF YEAR MEDICATION PICK UP- Medications need to be picked up by parent, guardian, or an adult at end of year. I will send a reminder letter home with information on dates and times for pick up. STUDENTS WILL NOT BE GIVEN MEDICATIONS TO BRING HOME. Any medication not picked up will be disposed of according to the NYS Department of Environmental recommendations.
Outdoor Recess
The students will go out for recess each day (weather permitting).Make sure your child comes to school dressed appropriately according to weather conditions. Please follow the Central Islip School District policy regarding our dress code. If your child comes to school inappropriately dressed the student’s parent is then responsible to drop off a change of clothes.
Screenings
During the school year, each child in Grades Pre K or K, 1, 3 and 5 will have a Vision Screening and Hearing Screening. New Entrants receive the Color Perception Screening. A height and weight assessment is sometimes completed also. Parents are only notified if there is a problem and are expected to make an appointment with your health care provider. The referral form must be completed and stamped by the doctor then returned to the school nurse ASAP.
- Kindergarten- vision and hearing
- 1st grade- vision and hearing
- 3rd grade- vision and hearing
- 5th grade- vision, hearing, and scoliosis (girls)
- 2nd and 4th provided by Primary Doctor.
Parents are only notified if there are any findings that would cause concern or need medical follow-up. A referral form will be given to the student to take home. Parent is expected to take student to their Health Care Provider. The referral form must be completed by doctor and returned to Health Office ASAP.
Sick Day Rules
Any child with a fever, rash, vomiting, or lice MUST be picked up from school and cannot go home on the school bus or walk home with out an adult. If child is sent home from school the child must be free of fever, rash, vomiting, and illness for 24 hours before returning to school. If your child is absent from school, due to illness for 3 or more days, a Doctor’s note is required and expected when child returns to school. Please report to Health Office with required Doctor’s clearance on the morning your child is able to return to school.
Any child with a contagious condition, broken/fracture bone, or needs to use crutches in school needs a note from their Heath Care Provider upon or before returning to school.
PARENTS AND GUARDIANS: Deciding to keep your child home from school is no easy decision. Use this guide to help determine if your child should take a sick day from school, as well as what to expect.
Fever: Children should not be in school if they have a temperature of 100?F or greater. Please keep your child home until fever is gone for 24 hours without the use of fever reducing medication. During flu season, children with a fever and other flu-like symptoms may be asked to stay home longer. Fever may be a sign of a more serious problem. Seek medical advice for fever lasting longer than 3 days.
Vomiting and Diarrhea: Children should not attend school if they have vomiting and/or diarrhea. Please keep your child home until 24 hours after the last time they vomited and/or had diarrhea. If vomiting and/or diarrhea continues and your child is unable to keep liquids down, you should seek medical advice.
Skin Rash: Skin rashes or irritation develop for many different reasons. Some are highly contagious and can cause serious problems. It is important to seek medical advice to determine the cause. In some cases, children may need to stay at home until the rash is gone. You are required to bring in a doctor’s note before returning to school.
Cough-Cold-Sore Throat: Children with productive coughing, sneezing, headaches, body aches, earache and/or sore throat may not be well enough to participate in school. Seek medical advice immediately if your child experiences difficulty breathing, wheezing or sudden high fever, chills and body aches.
Eye Irritation: Eye drainage, crusting, pain or redness may be a sign of infection and should be evaluated. If your child is diagnosed with an infection requiring treatment, he or she must stay home for 24 hours after treatment begins. You are required to bring in a doctor’s note before returning to school.
LICE: Your child will be sent home for lice treatment. Please report to Health Office the following day before the start of school with your child for clearance with the empty box of medication used for lice treatment.
Toilet Training
All children in kindergarten are expected to be toilet trained, to wipe him/herself and to dispose of the used toilet paper in the toilet bowl. However, accidents do happen; please send an extra set of clothes in a labeled bag. If your child accidentally stools on him/herself, you will be called to pick up your child.
